Residential Tenant Service Request

I, the undersigned, hereby request repairs or maintenance to be done in my apartment suite. I request that repairs or maintenance be attended to as soon as possible and for that purpose give you permission to enter my apartment. This acknowledgement shall operate as my consent, at the time of entry, for you to enter my apartment in order to effect such repairs, notwithstanding my absence at the time of such entry. I consent to you contacting me through any of the methods I provide above.
